HOW WE WORK
The Initial Call and Meeting
From the moment you call Nobilis, we begin considering you and your loved one's specific needs. On the initial call, we ask questions to form a basic needs assessment and to answer any initial queries you might have. We can explain what services we offer based on your needs.
During the call, we set up an in-home visit with you and the rest of your family – including the family member who may ultimately benefit from our support. This visit allows us to gather even more information, answer questions and let you understand how Nobilis can help.
The Personalised Care Plan
With Nobilis you will always know what services are being provided to your loved one. During the assessment, we develop a personalised care plan. This plan is tailored to you or your loved one's specific needs and gives detailed recommendations on exactly what kind of services we feel would benefit them.
This personalized care plan is fully discussed with you at the time to ensure everybody is happy with the suggested support.
We then write up the Care Plan and get it back to you for your consideration and agreement.
Support Worker Introductions
Once you approve the Care Plan, we search for the most appropriate person to implement care. Our system of matching support workers to clients takes numerous factors into consideration: services needed, your loved one's interests and the personalities of both your loved one and the support worker and their proximity to you.
By looking at the whole situation, we're better able to provide your loved one with the best support worker possible. At this point the Team Manager would visit with the key support worker to introduce them to you and to go through the services we are providing.
We do like to have a back up in case of illness or holidays, so we will ensure at least another support worker shadows your key support worker so they know you and the routine as well.
Of course, if there are ever any concerns about a specific support worker, we can search for and provide a replacement in very little time.
Support Supervision
Our support workers keep extensive notes about the care they provide and write it down after each visit in the Care Notes booklet. Those notes are kept in the home and are available at any time for any member of the family to review.
The following is also left in the home in a Nobilis folder:

Nobilis - Your Guide to services
We review your care package annually even if nothing changes but you can ask your Nobilis Team Manager at any time for a review of the support you receive.
