Reassurance and support for the family

Dorothy * is a 78 year old lady who lives alone in the house she has lived in for 45 years. She has no medical condition but was getting less mobile and found it difficult to get herself washed and dressed in the morning as well as she used to.
 
Dorothy’s daughter Hazel * was worried about her Mum. Dorothy’s neighbour used to drop in every other day but her health was deteriorating too. Unfortunately Hazel lives over an hour away so helping her Mum first thing in the morning and juggling her own young family was a struggle.
 
Hazel was recommended to Nobilis by Dorothy’s neighbour who had seen our Support Workers out and about in the area. Hazel viewed our website and called. During that initial call we talked through what concerns Hazel had about her Mum and we discussed areas we could help.
 
The Team manager went out to meet Dorothy, Hazel and her other daughter Ruth*the following day. She carried out a full assessment and expanded on the initial call to discuss what specific support we could give to maintain Dorothy’s independence. It was agreed that we would help Dorothy get up, washed and dressed each morning and a cold lunch prepared each week day.
 
Dorothy was introduced to two Support Workers later that week who would provide her care on a regular basis. A third was introduced a month or so later when Dorothy was comfortable with having support workers care for her, this allowed continuity of staff to cover holidays and sickness.
 
The support Nobilis provided allowed Hazel, Ruth* and the neighbour to spend quality time with Dorothy as opposed to coming over to wash and dress her. Dorothy is also pleased as she like to entertain and not be looked after by her daughters.
 
The family were very happy with the care provided and didn’t hesitate to speak to us about increasing her care when she became more unsteady following a fall.
 
We now support Dorothy twice a day for an hour in the morning and an hour in the evening. Once a week we take her out to a garden centre, for a coffee or to the library depending on how she feels to ensure Dorothy has company every day.
 

A peaceful and compassionate end to life

George* was an 87 year old gentleman who lived with his 60 year old daughter Felicity* who was his main carer and had been for a number of years.
 
It was after a series of hospital stays and an on-going battle with prostate cancer that Nobilis was contacted by the local hospice to provide support and respite to Felicity in the care of her Father.
 
It was George’s wish that towards the end of his life, if at all possible, he wanted to remain in his own home. He wanted to be in his own bed and cared for by his daughter. Felicity had provided this care and support for so many years she was determined to keep him at home right to the very end. She was exhausted!
 
An experienced Nobilis Team Manager sat with Felicity and the hospice team to discuss how best to support Felicity in the care of her father and to ensure he stayed as comfortable as possible, pain free and fulfilling his wishes to stay at home.
 
Within hours Nobilis staff were there to give Felicity a well needed afternoon off before a more structured Care Plan was agreed and planned with other organisations such as the Macmillan Cancer Support nurses.
 
George was supported 3 times a day initially but this soon increased to 4 times a day and night support 4 times a week as his condition became worse. Felicity was by his side most of the time so it was as much about giving her support and the opportunity to rest as it was to support George.
 
The team of support workers were well trained in palliative care and were able to make the last few weeks of George’s life comfortable and loving. By supporting George to bathe, turn, eat and ensure medications were accurate and timely, it gave Felicity the opportunity to spend quality time with her father in his last few weeks.
 
George passed away peacefully and calmly with his daughter at his side.
 

Male support and much needed respite for the family

Jonathan* is a 9 year old boy with severe Autism and ADHD. He lives at home with his Mum and younger sister and attends a school which caters for his needs in the week. Jonathan has limited verbal communication but is extremely active and can become agitated if does not have the opportunity to expend some of his energy.
 
Mum copes really well in the week juggling a full-time job, looking after Jonathan’s sister and keeping Jonathan as active and engaged as possible but the weekends and holidays were becoming increasingly challenging.
 
The Children Services team called Nobilis to provide some weekend support and perhaps additional support over school holidays. It became clear during the assessment with the Social Worker, Jonathan and his Mum, that Jonathan would benefit greatly from interacting with a male support worker. Somebody who could take him out in the community and assist Jonathan to do the things he enjoyed doing: playing football, going on long walks in the forest to climb trees, motor track racing. This in no way suggested that female carers could not support Jonathan in these activities it was just that he had female teachers, lived with his Mum and sister so his interaction with a male role model was limited.
 
Nobilis provides Jonathan with four hours of support every other Saturday and additional support in place during school holidays. These four hours are invaluable to his Mum and allows her to spend some quality time with her daughter.
 
We introduced Tom* who is fantastic with our younger clients and who immediately built a great rapport with Jonathan. He works alternate weekends so we schedule his work rota so he is always available to support Jonathan as change unsettles him. In order to avoid distressing change to Jonathan in the event of sickness and Tom’s holidays, we also introduced Peter* who supports Jonathan along with Tom in the school holidays so that they can cover for each other.

Ever four weeks we ask our clients to provide feedback on the support they are receiving, and on the performance of our care staff.  This allows all of our staff to understand where they can improve, but it is also really nice to see how appreciated they are for their work.  In addition to a "score" clients can comment on individual care colleague's and we have captured these to give you a feel for how well they are supporting our clients (obviously names removed for privacy)
